Sonic the Hedgehog 2 menyambung pengakhiran filem pertama dahulu, di mana di penghujung filem tersebut, Dr. Robotnik tercampak ke sebuah dunia asing dipenuhi dengan cendawan, yang menjadikannya semakin gila dan menukarkannya kepada susuk yang kita kenali dari permainan videonya. Selain itu, satu watak tambahan iaitu Tails turut muncul melalui satu portal di langit, dalam usahanya mencari Sonic di Bumi.
